Use this form to apply for, renew, or modify a Waste Discharge Permit required for facilities that discharge wastewater containing grease, oil, or other pollutants through a pretreatment device.

Application Information
Business Information
Provide the legal name of your business, as it appears on your business license.
Provide the customer-facing name if different from the legal name (e.g., store front name).
Business Operation Details
Describe the typical washing or maintenance activities that generate wastewater. Include where and how vehicles or equipment are washed, any indoor/outdoor wash areas, and whether wash water is recycled or discharged to the sanitary sewer.
Describe the type of food service provided. Some examples include “Medieval Sushi Tavern serving raw fish on wooden shields” … “Yoga Diner serving only stretch-inspired salads and meditation smoothies”.
Pretreatment Information
If you are renewing a WDP you have a PTD, you must mark Yes.
If unknown, provide dimensions (HxWxD).
Provide the physical location of the pretreatment device on the property (e.g., inside building, near wash bay, behind garage, or underground). Include any identifying details that help locate it during inspection, and describe which drains, wash bays, or areas are connected to the device.
If your facility has more than three PTDs, list all remaining units. Include the following details for each one:
A GRD is a Grease Trap or Interceptor. If you are renewing a WDP you have a GRD, you must mark Yes.
If indoors, specify whether the unit is in the floor or above it and identify the nearby fixture. If outdoors, specify which side of the building it is near.
If your facility has more than four GRDs, list all the remaining devices. Include the following details for each one:
Provide the name of the company that services/cleans your pretreatment systems.
List the primary cleaning products, detergents, or degreasers used in vehicle washing or maintenance. Include brand names if known.
If you are renewing a WDP you have a GRD and you create GRD waste, you must mark Yes.
If you cook food in oil you generate used cooking oil waste, you must mark Yes.
Provide the name of the company that services/cleans your GRD.
Provide the name of the company that picks up your used cooking oil.

APPLICANT FOR WASTE DISCHARGE PERMIT MUST READ THIS MATERIAL
BY SIGNING THE PERMIT APPLICATION BELOW THE APPLICANT AGREES:
The owner of every food service facility shall install or cause to be installed a pretreatment system for grease removal which meets or exceeds minimum sizing requirements of the Uniform Plumbing Code (SMMC 7.38.160(a)).
The owner of every automotive or vehicle maintenance facility shall operate and maintain pretreatment systems in accordance with SMMC 7.38.170.
