Please come prepared

The U.S. State Department's Passport website provides a list of forms and documents needed for a successful application.  Late arrivals and/or incomplete applications will require rescheduling.

Passports for children

  • Applicants ages 0-15 years - BOTH parents and applicant/child must be present.
  • Applicants ages 16 and 17 - only ONE parent and the applicant/child need to be present.

Passport fees are set by the U.S. State Department.  Only checks or money orders can be accepted for payment.  Bring two checks - one for the State Department for passport fees AND one for the San Mateo Public Library for the $35 execution fee and $15 photo fee, if applicable.
