Types of Applications Accepted:
- First-time passports
- Renewals for passports issued before the applicant was sixteen (16) years old. Copy of birth certificate is required.
- Renewals for expired passports issued more than 15 years ago.
- Replacement of lost, stolen or damaged passports
- All other renewals are handled via mail directly by the U.S. Passport Service.

Please Come Prepared:
If you have an appointment at the Library, you must complete the DS-11 form before arriving for your appointment for both new and renewal applications. Bring previous passport, if applicable.
Fees
Click Passport Fees to see the current U.S. State Department fee schedule. Only checks and money orders are accepted (no credit or debit cards). You will need two checks: one to the U.S. State Department for passport fees and one to the San Mateo Public Library for the execution fees and photo fees, as applicable.
The Library offers Passport Photo service for $15 per person.
