The use of the Participant Income Verification is optional and provided only as a resource. The data on this form must be collected, but may be collected via other participant intake processes.

Reporting Requisites
In the 2015-16 Program Year, HUD entitlement jurisdictions in San Mateo County worked cooperatively to implement a standardized, online application and report system for grantees and subrecipients. Grantee agencies are provided with their own user name and password to access their data and reports. The system is through City Data Services and can be accessed by clicking on the link below.

All quarterly reports are due by the 15th of the month following the close of the quarter; that would be the 15th of October, January, April and July, unless directed otherwise by the granting jurisdiction.
